3/4 butt shot of an assassin fly. They'll sit still for long periods but are very easily spooked

A bellows is effectively an adjustable extension tube... Extension tubes are fixed extensions and usually sold in sets... A bellows allows the user to adjust the magnification by racking the bellows in and out... Also used as a fine method of adjusting the focus points in very fine micrometer movements to create a series of images for focus stacking...

Yes... The majority of very close macro photography is done in manual modes because of the complexity of the process... It's generally done with stabilized (mounted/tripod) cameras in manual mode because of the very narrow depth of fields... Lighting is almost always augmented (even special lights) with manual exposure settings because the front elements are so close to the subject that normal lighting is blocked...
