Off flkr I found this:

"If you are using the CLS system and and the flash is used off camera this would be different. If it is on the camera then it can't read any return flash light as there is none from the Remote flash off camera."

It's it's to do with the flash fun not being able to tell the camera that it's fired off.

You dont say where when or how lol but Im going take a flying guess here Phil
My guess is its Flash.... Return light, meaning pre-flash !
Im guessing you have your pre-flash or anti red eye turned on.
The camera uses this info for determining flash exposure. Im gussing, enforced return light not detected, means your subject was out of range. :D

This is the EXIF I am talking about, the flash was the in build flash on camera. Does it just mean the flash fired? or did not fire?
