In my hand normally, its one of the original benbo and i tried a sling over my shoulder but it always had something pressing in my back or side, must admit i dont take it as often as i maybe should.
I carry a tripod around only if I need take photos of myself or during low light situation where I need to isolate/steady the camera. Otherwise, if I don't plan on taking any of those shots, I simply leave it at home.
I use a tamarac back pac that I lug around and I use a Monfroto MKC3-H01 that is small and light wieght. I just put the bottom of it in one of the external pouches and tie off the top of it to the top carrying handle of the backpac using a small bungy. Works great. I take it every where. When I dont, I need it. Never fails.
Like others, I only take the tripod with me when I anticipate a need. For short photo strolls through the woods I carry it on my shoulder, legs extended, ready for action.
I do own an Op/Tech tripod strap but haven't used it much. If I'm carrying enough gear to need one I would usually have a bag over my shoulder making an any additional shoulder strap somewhat awkward.
