Model release for iPhone

I need a simple model release that I can have on my phone. Snap a photo of the person and have them sign directly on the phone. I know I have heard of something like this but can't find it right now.

I have a personal project that will be ongoing for a couple of years and occasionally there will be people in the shot. I will want to publish some of these photos in a photo book at some point so release will be important.

Already bought Easy Release. It is really easy and does what I need it to do.

I'd still print some paper copies to hand to the model for records, and perhaps take a nice clean shot of it for yourself.
